You must complete your online registration.
Your must register and book your hotel reservations if you are traveling to to the forum.
An email confirmation will be sent when both your registration and hotel reservation is complete.
The forum will take place at the JW Marriott Camelback Inn Resort & Spa. To book your hotel use the "Book Now" link above, or contact the hotel directly at
800-582-2169. Please use reference "VillageMD event at JW Camelback Inn” when calling.
VillageMD will cover a maximum of 2 nights – Thursday and/or Friday. A rate of $275 per night has been negotiated, should you wish to extend your stay before or after the forum (Wednesday or Saturday). For extended stays, there is a daily resort fee of $35.00 and a $5.00 housekeeping fee per room per night. Please contact prior to booking to ensure availability for any extended stays.
Note: A credit card guarantee is required at the time of reservation. Your credit card will not be charged and is only used to secure your reservation.
After your registration has been received, you will receive a link from NexTravel. This custom link will allow you to book your air travel to the forum, which includes a economy fare ticket for participating attendees only. For those traveling with a spouse, air travel should be booked separately and not through the NexTravel site.
Ground travel from Phoenix Sky Harbor International Airport is a 20 minute drive. You can Uber, Lyft, or taxi to the resort.
By utilizing the above links, hotel and flights for participating attendees will be direct billed to VillageMD. All ground transportation will also be reimbursed. *Reimbursement instructions will be provided the final day of the forum.
To enhance the overall forum experience, we will be asking all of our attendees to download our mobile event app. This will assist us with onsite check-in and badge printing. As an attendee you will have access to interactive maps, live Q&A, networking within the app with other attendees, and so much more. All of the event details will be at your fingertips. Two weeks prior to the event, we will send instructions for the app download.

Kevin Ban, MD, is a physician executive with more than two decades of clinical experience and a track record for optimizing clinical programs, healthcare information systems, and patient safety. Prior to joining Walgreens as Chief Medical Officer, Kevin served as Chief Medical Officer at athenahealth, driving collaborative and strategic initiatives with a specific focus on population health. A board-certified emergency medicine physician with over 24 years of clinical experience, he served as Beth Israel Deaconness Needham’s CMO, and was a member of Harvard Medical School’s academic faculty from 2003-2016. Kevin led the Tuscan Emergency Medicine Initiative from 2003-2010 — driving success in emergency medicine education, quality improvement, translational research, and simulation training in the region — and launched the first pediatric trauma center in Italy where he served as Chairman of the program. Kevin received his BA and MD from Georgetown University, and completed his Emergency Medicine training in the Beth Israel Deaconess Harvard Affiliated Emergency Medicine Residency program.
